Structured Family Care
Structured Family Care is a caregiving service arrangement in which a Medicaid Waiver participate lives together with a caregiver who provides daily care and support to the participant based on the participants daily care needs and activities of daily living. This service arrangement allows the participant to stay in the comforts of their home while receiving care as an alternative to a nursing home facility.
If you or someone you know is caring for a family member or friend in the same household, you may qualify for financial compensation under the Medicaid Waiver program. Below is a guide that will assist you in navigating the process.

Participants are required to have Medicaid Waiver. If you do not possess Medicaid/Medicaid Waiver, we are eager to assist you in the qualification and approval process.

Upon approval, you will be assigned a Case Manager who will visit your home to conduct an assessment to provide a person-centered plan of care and determine the level of care, whether it be L1, L2 or L3.

Choose A Source of Life Home Care as your provider, and we will schedule an appointment to enroll you in the program.

We will perform an assessment and create a personalized care plan booklet, which will include your care plan, emergency contacts, medication list, and other pertinent details. This booklet will be kept in your home for easy reference.

You possess the freedom to choose your caregiver. Each caregiver must be at least 18 years old and complete training with our caregiver coaches. Moreover, caregivers will undergo ongoing training designed to meet the specific needs of both the participants and the caregivers themselves.

Caregivers receive a tax-free daily stipend, round-the-clock support from our dedicated care team, and up to 15 days of non-skilled respite care.

Once your caregiver has been identified and has met the necessary requirements, you can begin receiving care from your relative, spouse, children, or friend.
